Patient's and family have rights to access medical records.

Patient's and families have a right to information and education about health care needs.
Patient's has a right to be provided with information and access on whom to contact in case of an emergency.
Patient's has a right to information on organ donation.
Patient and Family have right to seek an additional opinion regarding clinical care.
Include determining what information regarding their care would be provided to self and family. In the case of minors, it will be provided to at least one of the parents/guardians.
Patient's Responsibilities
Provide complete and accurate information about his/her health. (Present, past medications, natural products, vitamins, allergies if any etc.)
Provide accurate and correct information including full name and address during admission or registration.
Abide by all hospital rules and regulations
Comply with no smoking & drinking policy
Comply with visitor’s policy to ensure the rights and comforts of all patients.
Be considerate of noise levels, privacy and safety.
Weapons are prohibited in the premises.
Treat hospital staff other patients and visitors with courtesy and respect.
To respect appointments if taken with treating consultant.
No self medication and also not to give medicines prescribed to him / her to others.
Provide complete and accurate information for insurance claims and work with the hospital and physician, billing offices to make payment arrangement.
To pay for services billed for in a timely manner as per the hospital policy.
To respect that some other patients medical condition may be more urgent than yours and accept that your doctor may be needed to attend them first.
To respect that admitted patients and patients requiring emergency care take priority for your doctor.
To follow the prescribed treatment plan and carefully comply with the instructions given.
To accept where applicable adaptations to the environment to ensure safe and secure stay in hospital. To ensure measure taken by the hospital, to ensure personal privacy, and confidentiality of medical records.
